Notice of Privacy Practices

This notice explains how I, as a mental health provider, can use and disclose your health information. You have rights regarding this information, including requesting access to it, and limiting how it can be shared. The provider cannot use your psychotherapy notes for anything other than your treatment without your written permission.

Informed Consent for Psychotherapy

Therapy is a personal and contractual relationship where we clarify expectations. Engaging in the therapeutic process may bring discomfort, but I will support you throughout. While your information remains confidential, there are exceptions such as threats of harm, abuse, or court orders. In public, I will not acknowledge you first to protect your privacy.

Practice Policies

This document outlines the policies for appointments, communication, confidentiality and termination of services with a mental health professional. It covers appointment cancellation fees, preferred communication methods and limitations of electronic communication. It also details confidentiality during teletherapy sessions and potential risks and benefits. Lastly, it covers termination of services and what to expect.
